आवेदन शुल्क:- आवेदन हेतु प्रत्येक पद के लिए सामान्य कोटि/अन्य पिछड़ा वर्ग के आवेदको को 400/-रू0 अनुसूचित जाति/अनुसूचित जनजाति के आवेदको के लिए 200/-रू0 का डिमाण्ड ड्राफ्ट आवेदन के साथ संलग्न करना अनिवार्य होगा। ड्राफ्ट का भुगतान जिला ग्रामीण स्वास्थ्य समिति (संबंधित जिला का नाम) के नाम से संबंधित जिले में भुगतेय होगा।
